Glenoid bone loss has been associated with both simple anterior dislocations as well as complex and revision shoulder instability cases. With an increased number of dislocations, the likelihood of significant glenoid bone loss increases. Identifying and treating glenoid bone loss that is related to acute or chronic shoulder instability is critical to successful stabilization surgery. Drs. Provencher and Solomon present a video to help the viewer identify normal glenoid bone contour. They then provide a reliable method for quantifying glenoid bone loss and glenoid labral articular defects. The video begins with the normal arthroscopic appearance of the glenoid and then presents a method to identify and quantify anterior-inferior glenoid bone loss. It also provides a brief review of the literature on recurrent instability related to bone loss and discusses the ramifications of the choice of surgical procedure. Tips for arthroscopically treating bone loss of less than 20-25 per cent of the glenoid diameter complete the video. After watching this video, the viewer should be able to identify glenoid bone loss and understand when arthroscopic treatment is a viable option and when to augment the glenoid with iliac crest autograft or a coracoid transfer.
