Based on legislation enacted in 1989, the new Medicare Fee Schedule (mfs) for physician services was put in place on January 1, 1992. The mfs replaced a pay ment system under which medicare's rates were set separately for each physician at the lowest of the actual charge, the physician's customary charge, or the locally pre vailing charge for that service.
