The 2017 performance year Accountable Care Organization (ACO) directory is based on the Centers for Medicare and Medicaid Services (CMS) released data. It includes key member data for all 480 ACOs active during the year. ACOs are authorized under the Medicare Shared Savings Program (MSSP) as part of the Patient Protection and Affordable Care Act (PPACA or ACA). This work is divided into two main sections, listings of ACOs-alphabetically and by state-as well as member information for each ACO, including: ACO name and CMS ID Street address Web site Market service area(s) Authorization status and model type Executive contact: name, phone number, email address Public contact: name, phone number, email address This volume is the companion to the Point!Guide ACO Performance Report which provides key financial and operational results for each ACO along with county-level market share information.
