Incomplete as the work is in many respects, yet, owing to my nu merous professional engagements, I could not have collected the materials on which it is founded had it not been for the assistance I have received from many friends. It would be difficult to assign to each the due amount of my Obligations, and I must therefore content myself by thanking them collectively. There are a few, however, whose names cannot be altogether omitted. Thus, I am especially in debted to my colleague, Dr. Semon, who has prepared a German translation of this work (to appear simultaneously with the English edition), for many important suggestions and much keen criticism; I have to thank my former assistant, Dr. Gordon Holmes, for valuable aid in matters of historical research, more particularly in connection with those authors whose Observations are recorded in the classical languages; and I am very grateful to Mr. Mark Hovel, Resident Med ical Officer and Registrar to the Hospital for Diseases of the Throat and Chest, for preparing a detailed index.
