From the FOREWORD:
THIS Index is limited to the Characters, or Persons, who have Speaking Parts assigned to them in the First Folio, and records the first occasion on which they speak, not necessarily always their first appearance in the drama.
The spaces between the entries are for the purpose of inserting the previous or subsequent appearance of the dramatis personae, thus rendering the Index adaptable to any edition of Shakespeare's Plays.
Where the Plays are not divided into Acts in the First Folio, the page in that section is indicated by larger figures.
